The patient is still in a lot of pain"
The patient continues to report significant pain"
Despite regular analgesia as charted, the patient continues to report significant pain (7/10), which may warrant a review of the current pain management regimen"
分主题积累法——按转介请求类建立功能短语库:转介理由("In light of the patient's deteriorating...")、风险评估("poses a significant risk of...")、合作性请求("I would appreciate your input on...")。
Please check her blood sugar because it's been quite high lately."
I would appreciate your monitoring of Mrs. Chen's blood glucose levels, given the elevated readings observed over the past three consecutive days (12-15 mmol/L preprandially), which may indicate a need for adjustment of her current hypoglycaemic regimen."
The patient reports progressive dyspnoea over 72 hours, accompanied by purulent green sputum, strongly suggestive of an infective exacerbation of COPD. Despite increased salbutamol use, his breathlessness has not improved, raising concerns about inadequate bronchodilation. A sputum culture, chest X-ray and respiratory physician review are recommended."
The patient could not breathe properly, and this caused his oxygen saturation to fall, so we had to start oxygen therapy immediately."
The patient's acute respiratory compromise, evidenced by a significant desaturation event, necessitated the immediate commencement of supplemental oxygen therapy."
The patient's suboptimally controlled type 2 diabetes (HbA1c 9.8%) necessitates a multidisciplinary approach. A referral to dietetics for tailored nutritional counselling and to podiatry for regular foot surveillance are therefore recommended as priority interventions."
